OA ELECTION FORMS

The following  forms are in two formats.  You may use the Word format to complete or the Adobe Acrobat PDF document to print and fill in by hand.  The Chapter and Lodge would prefer to have a completed one with signatures but would appreciate a digital copy of any word document you may complete

Choctaw Chapter Ballot Form - This is a sample of a form used by one of the troops during our election process.  It has been the best example of providing information about scouts being considered for the Order of the Arrow.

UNIT ADULT CANDIDATE RECOMMENDATION - Troop leaders should make certain that the OA Advisor is provided a copy of this form.  This form MUST be submitted to the Youth Election Chairman. This SHOULD be done through the Chapter Chief or Chapter Advisor.  The PDF form is an online FILL IN THE BLANKS form.  After you complete it simply print.  (Please note you can not save the form with the filled in information, you can only print it)  Units may have one adult for each 50 registered Scouts or fraction thereof, provided at least one youth is elected in the Troop. UNIT ELECTION FORM - This form should be completed prior to the closing of the Unit election.  BE SURE that addresses are correct as this will be the only means that a youth will be notified to attend Ordeal and complete his membership

OA TROOP REPRESENTATIVE Info Document - this is an extensive document with all the information and resources needed for a Scout or Scoutmaster wanting to learn about this position.

OA Lodge Application - (INTERACTIVE DOCUMENT) this Form is for members transferring from other Lodges into the Chilantikoba Lodge # 397 OR for members seeking reinstatement into Chilantikoba Lodge.  It can also be used to Renew expired membership.  If you desire the form in word format use the following link - WORD.

Choctaw Chapter Meeting Agenda  - Our meetings are planned so we can get to the fun as early as possible.  This form is used by the officers to make things happen!

Cypress District Web Permission Form All Officers, Adult Advisors, and members should complete and mail this form so that honor recognition on the web will be more easily executed.

Jacket Order Form (230KB Adobe Acrobat File)-  All preorders should be in by September 3 in order to be completed by the Fall Fellowship.
